All patients have the RIGHT to:

  • Treatment, regardless of age, race, color, creed, sex, ethnicity, national origin, marital status, sexual orientation, handicap, religion, or method of payment.
  • Be treated as an individual, to be evaluated according to their needs, and to be given assistance necessary to provide for their personal needs.
  • To be treated by considerate, respectful and competent staff who function according to a strict professional code of ethics.
  • Have their personal dignity recognized and respected.
  • Have the nature of care, procedures, and treatment they will receive explained to them in a language they can understand.
  • Confidentiality. All records concerning their treatment will be kept confidential and released only with their written permission or if specifically required by law, as outlined in the Notice of HIPAA Privacy Practices.
  • Refuse medication, treatment, or offered procedure. A refusal may become a treatment issue and they may be discharged or referred elsewhere for care.
  • Be oriented and informed about practice rules and regulations.
  • Examine and receive a detailed explanation of their bill.
  • Information and counseling on the availability of known financial resources for their care.
  • Request an option of a consultant at their expense or request an in-house review of the patient treatment plan.
  • Access their records, in accordance with the Notice of HIPAA Privacy Practices.
  • Initiate a complaint or grievance procedure and the appropriate means of requesting a hearing or review of the complaint.
  • Transfer to another provider or treatment facility when appropriate and acceptance by the provider or treatment facility has been determined.
  • Receive reasonable continuity of care and to have a continuing care plan for further treatment and/or health care needs in place at time of discharge.

All patients have the RESPONSIBILITY to:

  • Provide accurate information about their health and addiction issues.
  • Abide by the patient controlled substances agreement.
  • Provide updated, current contact and insurance information.
  • Work on the continuing treatment plan as developed with the physician and treatment team.
  • Talk to their physician if they do not understand or have questions about their treatment.
  • Arrange for the payment of their treatment.
  • Follow the rules and safety regulations.
  • Keep appointments and let staff know at least 24 hours in advance when it is not possible to keep an appointment.
  • Respect the privacy of other patients and maintain the confidentiality of others.
  • Treat the staff with the same respect and consideration provided to the patient.
