COMMON REFERRAL REASONS

  • Chronic pain with opioid or polypharmacy concerns — patients whose pain is difficult to manage safely because of long-term opioid use, multiple controlled medications, or complex medication regimens that raise concerns about side effects, interactions, or overall function.

  • Difficulty tapering opioids safely — patients who want or need to reduce opioid use but have struggled with worsening pain, withdrawal symptoms, anxiety, or failed taper attempts in the past.

  • Co-occurring pain and substance use disorder — patients living with both active or prior substance use disorder and chronic pain, where treatment requires a careful, coordinated approach that addresses both conditions without worsening either.

  • Patients cycling through acute care settings — patients with repeated emergency department visits, hospitalizations, or urgent care use related to pain crises, medication issues, withdrawal, or unstable symptom management, who may benefit from a more consistent outpatient plan.

Payment Policy:

Sisu Health does not bill insurance. Payment is due at the time of the appointment, and patients will receive the documentation needed to seek reimbursement if their insurance plan offers out-of-network benefits. Since reimbursement policies differ by plan, we encourage patients to confirm their coverage directly with their insurer in advance.
