COMMON REFERRAL REASONS
Chronic pain with opioid or polypharmacy concerns — patients whose pain is difficult to manage safely because of long-term opioid use, multiple controlled medications, or complex medication regimens that raise concerns about side effects, interactions, or overall function.
Difficulty tapering opioids safely — patients who want or need to reduce opioid use but have struggled with worsening pain, withdrawal symptoms, anxiety, or failed taper attempts in the past.
Co-occurring pain and substance use disorder — patients living with both active or prior substance use disorder and chronic pain, where treatment requires a careful, coordinated approach that addresses both conditions without worsening either.
